This position will be located in Dallas/Forth Worth, TX.

Job Overview

The Nurse Practitioner will have responsibility of managing a small panel of patients with multiple, complex conditions, in partnership with a nurse care manager (RN). The role is responsible for delivering high quality clinical care and best-in-class experiences and for our patients. Reporting to the VP, Clinical Operations, the Nurse Practitioner will be highly organized, detailed-oriented and willing to roll up his/her sleeves to maintain our exceptional quality of care and ultimately improve patient outcomes.

Job Responsibilities

  • Perform comprehensive initial assessments for newly enrolled patients. The NP will flex between house calls and clinic visits in addition to guiding the Virtual Clinic nursing staff with complicated oncology matters. S/He will manage patient care on a longitudinal basis, delivering proactive care to reduce unnecessary hospitalizations. Diagnose, prescribe and treat patients, consistent with standard of care. Partner with the patient’s oncologist, primary care provider and other specialists to deliver integrated care. Participate in a rotated on-call program for the home-based patient support after-hours. Efficient and thorough charting is key in this role.
  • Deliver high quality clinical care to patients through the Virtual Clinic platform. Answer chats, calls (voice and video), triage symptoms, and coordinate care plans with the office-based care team. Proactively communicate with patients throughout their journey, increasing adherence to medication and treatment plans, as well as improving the overall experience. Develop and deliver evidence-based care protocols and steer care to highest quality and lower cost care providers.

  • Proactively initiate and drive change in clinical protocols, processes, tools and capabilities. Identify training opportunities, workflow enhancements, new patient-facing capabilities, and new measurement tools.

  • Build and foster relationships within a matrix organization structure to proactively resolve issues, drive efficiencies, and champion the service-centric strategy in a healthcare delivery environment.

Job Requirements:

  • Master of Science in Nursing (MSN), Nurse Practitioner (NP)
  • Advanced Nurse Practitioner (ANP) in state of practice and DEA (current unrestricted license)
  • American Heart Association Basic Life Support (BLS) Certification
  • 5+ years of clinical NP experience
  • Knowledge of oncology clinical standards of care
  • Oncology care management experience strongly preferred
  • Willingness to conduct house calls
  • Current state driver’s license
  • Experience in a remote environment is a plus
  • Strong computer skills and comfort using several programs at once
  • Passion for delivering care to patients with cancer and other complex, chronic illnesses
  • Entrepreneurial, flexible, self-starter
  • Excellent communication skills, verbal, written and presentation to effectively facilitate remote resources
  • Tenacity and relentless energy in developing solutions amidst ambiguity and complexity
